Questionnaire below. At National E&S, we require that
each retail broker be appointed. When completed, you may
fax or email these forms to National E&S Insurance Brokers,
Inc. at (661) 266-9390 or cesar@nationaleands.com, but you
must immediately send the originals to:

The process is easy:
Upon receipt of all of the necessary material listed above,
we will begin processing your appointment. Upon receipt
of your completed paperwork, National E&S will review
all information. Should any additional information be needed
or not provided, a Marketing Specialist will contact your
office. The Marketing Specialist is your resource for information
about our products, our markets and our staff, and will
assist you in developing a positive and rewarding relationship
with your National E&S Broker.
